Responsibilities
- 01Undertake initial holistic nursing assessments and develop individualised care plans in partnership with the client, their family and the care team.
- 02Effectively communicate with the care team on the clinical condition or diagnosis and treatment or discharge plans of clients, ensuring accurate records are maintained.
- 03Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation.
- 04Support the care team by delivering client-specific clinical training, mentorship and supervision by providing feedback, conducting performance reviews, and supporting professional development.
Requirements
- 01A learning disabilities nurse with an active NMC pin
- 02Experience leading community teams to support individuals with learning disabilities, autism and mental health needs
- 03A valid drivers license and access to a vehicle
