Обязанности

01Undertake initial holistic nursing assessments and develop individualised care plans in partnership with the client, their family and the care team
02Effectively communicate with the care team on the clinical condition or diagnosis and treatment or discharge plans of clients, ensuring accurate records are maintained
03Identify and manage risks as part of care provision and lead the investigation and resolution of clinical incidents, ensuring timely and thorough documentation
04Support the care team by delivering client-specific clinical training, mentorship and supervision by providing feedback, conducting performance reviews, and supporting professional development

01A learning disabilities nurse with an active NMC pin
02Experience leading community teams to support individuals with learning disabilities, autism and mental health needs
03A valid drivers license and access to a vehicle
